Become a COVID-19 Vaccine Provider!

POMA is working with the Pennsylvania Department of Health (DOH) to spread the word that YOU can become a COVID-19 vaccine provider.

Below is a message from Pennsylvania's Acting Physician General, Denise Johnson, MD to POMA members regarding this important initiative:

Thank you for the extraordinary work you have done over the last year to serve and protect our communities in these difficult times. Some of you have held vaccine clinics for your underserved neighbors, others have worked daily to continue to provide your patients with high quality care. The Commonwealth of Pennsylvania is healthier and safer because of your dedication and service.

I am writing to you now because the fight against SARS-CoV-2 is not yet over and Pennsylvania still needs your help. Specifically, the county you serve currently has one of the lowest rates of vaccinations across the state.

I am personally asking you as a public health official and fellow physician to help us spread the word about vaccine safety, availability, and effectiveness to your communities. Can you please join our work by participating in one of the following ways?

    • Enroll as a provider and administer the vaccine in your office – access enrollment form here
      • Note: You must use this enrollment webform, which must be 100% complete (including all license numbers and freezer storage estimates)
    • Actively encourage your patients to get the vaccine through a phone call or when they come in
    • Hand out vaccine educational materials to your patients when they visit your office – order materials here or click here to download images
